fa احساس عدالت اجتماعی در مراکز درمانی سازمان تأمین اجتماعی: نقش عوامل نهادی و زمینه‌ای با در نظر گرفتن زمینه فضایی Perceived Social Justice in Social Security Organization Healthcare Centers: The Role of Institutional and Contextual Factors within a Spatial Context نابرابری سلامت اصیل orginal مقدمه: درگیری و زد و خورد بین کادر درمان و مراجعان به مراکز درمانی، چالش اساسی است که هر ساله هزینههای مادی و معنوی فراوانی را بر کشور تحمل میکند. بخش زیادی از این چالشها به عدم رعایت انصاف و عدالت مربوطند. این پژوهش در همین راستا و برای تعیین وضعیت احساس عدالت در مراجعان به بیمارستانهای تامین اجتماعی مشهد، به انجام رسید. روش: روش تحقیق توصیفی و تحلیلی و جامعه آماری، بیمارستانهای تامین اجتماعی و 300 نفر از مراجعان بودند. دادهها از طریق برداشتهای میدانی و نظرسنجی گردآوری شد. تجزیه و تحلیل دادهها با استفاده مدل پی-مدین،‌از نرم افزار GIS و رگرسیون خطی چند متغیره در نرم افزار SPSS انجام گردید. یافتهها: یافتهها نشان داد که تنها دو بیمارستان تامین اجتماعی در مشهد فعال است و بخش قابل ملاحظهای از جمعیت شهر برای بهرهمندی از این مراکز، هزینه مادی و زمانی زیادی مصرف میکنند. همچنین احساس عدالت در بین 38 درصد از پاسخگویان در حد کم و خیلی کم، برای 14درصد در سطح بالا و در نهایت برای 48 درصد بقیه در سطح متوسط است. عوامل تبیینکننده احساس عدالت بر طبق مدل رگرسیونی، متغیرهای اعتماد به کارایی پرسنل با ضریب 58/0، سواد با ضریب 21/0، احساس رعایت انصاف با ضریب 17/0 و احساس امنیت با ضریب 12/0 دارای تأثیر افزاینده میباشند و متغیرهای احساس بیگانگی با ضریب 104/0- و محله مسکونی نیز با ضریب 099/0- دارای تأثیر کاهنده هستند. در مجموع، این متغیرها 53/0 درصد از تغییرات متغیر وابسته را تبیین میکنند. بحث: یافتههای این مطالعه مشخص ساخت که مراجعان، کیفیت درمانی مراکز را بیش از توزیع فضایی آنها مدنظر دارند. بنابراین، قبل از تصحیح پراکنش مراکز، بایستی نسبت به تجهیز بیمارستانها بر اساس نیاز مراجعهکنندگان اقدام کرد. Introduction: Conflict and clashes between medical staff and patients visiting medical centers are a fundamental challenge that imposes significant material and moral costs on the country every year. Many of these challenges are related to a lack of fairness and justice. This study was conducted in this regard and to determine the status of the sense of justice among those visiting social security hospitals in Mashhad. Method: The research method was descriptive and analytical, and the statistical population was social security hospitals and 300 clients. Data was collected through field observations and surveys. Data analysis was performed using P-Median model, GIS software and multivariate linear regression in SPSS software. <div style="text-align: justify;">Findings: The findings showed that only two social security hospitals are active in Mashhad. A significant portion of the city's population spends a lot of money and time to benefit from these centers. Also, the sense of justice among 38% of respondents is at a low or very low level, for 14% at a high level, and finally for the remaining 48% at a medium level. Factors explaining the feeling of justice According to the regression model, the variables of trust in personnel efficiency with a coefficient of 0.58, literacy with a coefficient of 0.21, feeling of fairness with a coefficient of 0.17, and feeling of security with a coefficient of 0.12 have an increasing effect. The variables of feeling of alienation with a coefficient of -0.104 and residential neighborhood with a coefficient of -0.099 have a reducing effect. In general, these variables explain 0.53 percent of the changes in the dependent variable. Discussion: The findings of this study revealed that clients consider the quality of treatment of centers more than their spatial distribution.
